Referrals

If you have any important history or current health conditions, a referral can be very helpful. They can help me to understand what's going on for you and assist me to provide a holistic service. Referrals also assist in continuity of care between healthcare providers. 

If you are seeking a Medicare rebate and you qualify for relevant funding (as determined by your GP), you will require the associated formal referral.

If you're are a Participant in the NDIS, please bring a referral including a list of other service providers.

With your permission, to ensure clear and consistent communication with your healthcare team, I will liaise with your other healthcare providers.
